Instructions

For each parent, staff and student representative election (including all by-elections) please complete the form below:
  • Note: In the Triennial elections this form must be completed for each election type, ie. one for the parent representative election, and one for the staff representative election.
  • Note: Candidate and Election Results - Appendix 1 replaces Appendices A to E used in previous elections.
  • Note: If you received nominations for more than 25 candidates, you will need to complete the form twice (or more as applicable). These will be collated by the Ministry of Education.
  • Note: If the election is for a combined board. Please enter name and number for one of the schools.
