The following survey was adapted from the work of Mary J. Baker-Ericzen, Mary Garanand Muggenborg and Mary M. Shea as documented the the article "Impact of Trainngs on Child Care Providers' Attitudes and Perceived Competence Toward Inclusion. (2009) Topics in Early Childhood Special Education.
