Washington State Multistate Nurse Demographic Data Survey

In accordance with Substitute Senate Bill (SSB) 5499 Multistate Nurse Licensure Compact, as a condition of employment by certain entities, individuals that hold a multistate nurse license issued by a state other than Washington, are required to complete this demographic data survey as required by the Washington State Board of Nursing.
