The following short survey is for families and professionals. The survey is intended to identify areas in which families and practitioners need additional information about programs for children who are deaf/hard-of-hearing, such as Virginia's Early Hearing Detection & Intervention (EHDI) program. This survey is very important in our mission to improve outcomes for children with hearing loss.
Survey responses are anonymous and confidential. Results will be used to plan for professional education and for creating materials and supports for families.
At this time we are only collecting information from professionals who serve infants and young children who are deaf/hard of hearing (ages birth to 5 years old) and from parents of young children who are deaf/hard of hearing.
