2025 AHHC Paraprofessional of the Year Award - Nominee Profile Form

Criteria for the Award:
This award is designed to recognize paraprofessionals within the home care, home health, and/or hospice care industry. The nominee must have a minimum of one (1) year experience working in home care, home health, and/or hospice. Appropriate nominees may be, but are not limited to the following positions: Habilitation Technician, COTA, PTA, LPN, Pharmacy Technician, DME Equipment Technician.

They must demonstrate:
- A commitment to the industry by serving on internal agency committees, AHHC committees or community involvement;
- An interest in professional growth through continuing education;
- Excellence and commitment to patient care; and Dedication to raising the. level of professionalism.
