Before You Begin
In order to complete a nomination, nominators will be asked to provide contact information for nominee(s) and nominator in addition to providing nomination materials.  These materials include a required nomination statement and resume/bio for the nominee; optional materials include up to three letters of support and supplemental material. 

Please note: Responses throughout this form will only be saved upon submitting a nomination - nominators are not able to save their progress and return to the form later to complete their nomination.  Therefore, it is recommended that nominators have all submission information and materials ready when beginning the nomination form.

Nominations will be evaluated based on the award criteria as described in the program guidelines and based on consideration of the nomination materials. The nomination statement and letter(s) of support may be submitted as written, audio, or video formats. Letter(s) of support and supplemental material are not required for a nomination to be eligible. Please note that nominations of co-leaders must be completed as a single nomination submission.
