Wellness Policy Plan

The Concord School District has begun the process of updating our district-wide wellness policy.

A strong wellness policy helps to create supportive school nutrition and physical activity environments that promote better attendance, improve behavior and increase academic performance.

To begin our process, we would like to assess current activities and policies in each Concord school.

The survey is divided to target individuals to be able to assess physical activity and health; nutrition, administrative aspects for wellness and one for all staff to complete regarding general aspects of wellness.

Each survey should take approximately 15 minutes to complete.
