You are invited to participate in an online survey for a research project conducted through Mercer University. Mercer University’s IRB requires investigators to provide informed consent to participate in this project.

The purpose of this online research study is to examine the healthcare needs, barriers and interests of rural populations on receiving services from the Health Equity Navigator Program. Your participation in the study will contribute to a better understanding of how to support the community in accessing the healthcare system in Georgia. You must be at least 18 years old to participate.

If you agree to participate
The survey will take approximately 10 minutes of your time. You will complete an activity about accessing health resources and services. You will not be compensated, however, we will give you our promo items for your participation.

Risks/Benefits/Confidentiality of Data
There are no known risks or discomforts which could cause you to feel uncomfortable, distressed, sad, tired or any injury. There will be no costs for participating. Although your participation in this research may not benefit you personally, it will help us understand how to serve and disseminate information to communities in underserved and rural Georgia. Your name and email address will not be kept during the data collection phase because it is a one-time survey. A limited number of research team members will have access to the data during data collection. The final data will exclude your name and contact information, such as your residential and email address.
