DNR Landfill Fire and Leachate Release Notification (Form 4400-012)

Landfills are required under s. NR 506.17 (5), Wis. Adm. Code, to notify the Wisconsin Department of Natural Resources, in writing, within a specified time period when a leachate release or fire occur. Completing this form will fulfill the notification requirements. The associated information will be relayed to the assigned DNR solid waste engineer, who will follow up, if warranted. We encourage landfills to reach out directly to their assigned DNR solid waste engineer with questions or when these events significantly affect operations.
