2015 NW Arkansas Great Arkansas Workout

School Application Form

This is an application to participate in the NW Arkansas Great Arkansas Workout on May 1, at the Walton Life Fitness Center, 1701 SE 14th Street, Bentonville, AR. Applicants will be notified whether or not their school is accepted to attend the event.

Special Note: The Great Arkansas Workout is limited to fourth grade students.

Do you have an active school wellness committee that meets at least twice yearly?(Required.)
Have you ever participated in the Great Arkansas Workout? (Prior participation is not included in the selection process.)(Required.)
