WA Mental Health Awards 2026 - Nomination Form

The Innovation in Service Delivery Award recognises a person, team or organisation operating in the mental health sector, which has developed a new or innovative approach to service delivery. The new approach should be transformative for the mental health sector and/or improve the lives of people with lived experience of mental health challenges.

The new approach may be a result of feedback from people with lived experience or the result of co-design. The successful nominee can provide clear evidence of the change in the service delivery and the impact on Western Australians.
Submitting a nomination for the Innovation in Service Delivery Award

Nominations for this award are open to any person, team or organisation operating in the mental health sector. Self-nominations are accepted for this category.

Please address the following questions in a separate document and upload as an attachment to this form:
  • Please describe the new or innovative approach to service delivery, including how it evolved, what it includes and the key stakeholders involved.
  • What issue, challenge or need does it address and why is this important?
  • Please explain how this approach is different to what has been done before?
  • How does this initiative have the potential to be transformative for the sector or make a meaningful difference to the lives of Western Australians?
The written response to the above questions should not exceed 2,500 words, excluding supporting documents or images.
