Thank you for accepting our invitation to be included on our cis and trans gay, bisexual, and queer (GBQ+) friendly directory of primary care providers, including physicians and nurse practitioners. It is partners such as yourself that aid in shaping a healthier, stronger community.

The directory will be composed of a private internal directory that ACT and allied service organizations will use to directly refer clients to you.

You may also be listed on the public directory of LGBTQ+ friendly healthcare providers of Rainbow Health Ontario (RHO), an initiative of Sherbourne Health Centre
