People's Music School Family Directory (2026-2027) |
Purpose
To support access and communication among families, TPMS offers an opt-in Family Directory that may be used by families to coordinate transportation to and from TPMS programs. Participation is entirely voluntary.
If you would like to have your contact information distributed to other parents who have opted in to a TPMS contact directory, please fill in the information below by 9/7/2026. This directory will be distributed via pdf to the phone number or email address requested at the bottom of the form on 9/8/26.
By opting in, I agree to share my contact information with other TPMS families.

I understand and agree that:
- Any transportation arrangements made between families are private, voluntary, and at my family's own risk.
- TPMS does not organize, supervise, vet, or assume responsibility for any transportation coordination, including carpools.
- TPMS shall not be held liable for any accidents, injuries, delays, or disputes that arise from transportation arrangements among families.
- TPMS shall not be held liable for any consequences of information being shared by participants and non-participants in the voluntary program.
- TPMS does not verify driver eligibility, insurance coverage, or vehicle condition for families participating in carpooling.
- Any related matters shall be governed by the laws of the State of Illinois and I will hold TPMS harmless from any issues or disputes arising out of this communication.
