Welcome to the VCSQI SAN 2.0 Online Registration System. Our assistance is free to your practice since it is part of CMS's national Transforming Clinical Practice Initiative (TCPI). TCPI is a federally-funded project open to your practice's voluntary participation. Joining the program will help your practice thrive in the rapidly changing healthcare landscape!


By now we hope that you have addressed the 2017 Quality Payment Program (QPP) reporting requirements (MIPS) since the deadline for submission was April 3rd, 8:00 P.M. E.S.T. We can help you comply with 2018 Medicare QPP reporting requirements AND position your practice for a positive payment adjustment. The 2018 QPP requirements are less flexible than those for 2017 and will also now include the Cost category along with Quality, Improvement Activities and Advancing Care Information. Our coaching over the next several months will help you to improve quality, eliminate waste, reduce cost and thus optimize your performance and payment adjustments for future years.

Through your participation in the TCPI, VCSQI will assist your clinicians of any specialty and in any state to improve quality, lower costs and engage patients and family members in their care. Again, VCSQI has federal funding to offer this help and therefore it is free of charge for your practice to participate. All you need to do is complete the following participation form and we'll get started right away.

Practices participating in the VCSQI SAN 2.0 will get a specific plan for supporting the needs of their practice, with a focus on identifying areas for improving patient care, practice team functioning, and the financial health of the practice. In addition to this customized plan, participating practices will also receive the following benefits:

  • Technical assistance and access to free, CMS-developed resources for quality and process improvement
  • Hands-on coaching with your practice's quality data to set goals and track progress
  • Focus on Person and Family Engagement (PFE), with resources developed to maximize patient activation
Improved health of your clinicians and practice:
  • Opportunities for networking and learning from national leaders in practice process improvement and value-based care
  • Assistance to support you and your practice on your path to value-based care
  • Help to maximize current CMS payment incentives and to avoid financial penalties, such as those from the Merit-Based Incentive Payment System (MIPS)
  • Coaching on how to select an Alternative Payment Model (APM) that is right for your practice
