The New Hampshire Department of Health and Human Services' Health Professions Data Center collects key practice and capacity data from all licensed dentists in the state during license renewal. This survey is designed to collect general information regarding your current provider status and the locations at which you routinely practice dentistry.

The data is a key resource in statewide health care workforce assessment and health care access planning, which informs emergency preparedness, workforce pipeline programs, and policy.

The data collected is confidential and protected; it will only be used for the aforementioned purposes.

Only dentists due to renew an active status license in 2025 should complete this survey.

The survey is a requirement of license renewal and is complete when all responses are submitted by selecting "Done" at the end of the survey.
