You are invited to be a part of the research described here. Participation is voluntary and you may stop at any time. The survey questions will be easier to read on a computer or tablet than a phone; this is a good time to switch if you are reading this with a phone now. 
Summary of the study: This research is looking at what factors affect quality of life and function in people with various chronic health conditions compared to people who are healthy.
You will be asked to answer questions online via SurveyMonkey. The survey should take approximately 45 minutes. Your answers will all be anonymous. The questions will be similar to ones asked by healthcare providers, and will include questions about your quality of life, function, pain, self-awareness, fatigue, quality of sleep, stress and mood. Because the data collected are entirely anonymous, there is no risk of your personal information being released.
Because it is difficult to recruit similar people who do not have chronic illnesses, we are also asking each participant to consider asking one healthy person, preferably of similar age and lifestyle to participate, but this is not required. All participants must be at least 18 years old and be able to read and respond in English.
