This information collection tool will help identify the needs of individuals/families caring for an adult with special needs in the Prescott-Russell region.  

We would like to hear your thoughts as we work on determining the services needed in our region for mentally challenged individuals.

We are aware that after school, many mentally challenged adults are limited in terms of access to activities that are appropriate for their needs. This project aims to find ways to address that situation and to try to make the lives of those young adults more pleasant and interesting while enabling their families to attend to their day-to-day activities.

The purpose of the following questions is to determine individual needs, but in very general terms, for each adult and each family.

You do not have to answer every question. Feel free to skip those that are not applicable or that involve information you prefer to keep confidential.

At the very end, there is a section where you can provide additional information in case we have not covered certain concerns you would like to share, or simply ideas about services or activities that might interest you.
