Two Dates to Know
  • Submit this application form by December 28, 2026.
  • Complete all three assessments and upload your results PDFs by January 4, 2027.

After submitting this form, you’ll receive your DISC access code by email and be directed to instructions for all three assessments. Plan for approximately 45 minutes to complete the assessments and submit your results.
