Error management is an essential part of the total quality management system in Medically Assisted Reproduction (MAR), however, the data on how to tackle this in MAR is scarce. ESHRE is dedicated to filling this gap with good practice recommendations on how to deal with errors in MAR. To be able to give a good overview of the current practices, we kindly ask you to participate in a survey related to aspects of error management.

The purpose of this survey is:
(i) to examine how centres for medically assisted reproduction manage the errors of clinical and laboratory staff within the centre;
(ii) to understand how errors are reported to national Competent Authorities (CA);
(iii) and if and how errors are ethically disclosed to patients.

The results of this questionnaire will be summarized in the recommendation paper that will be published in HROpen in the section of the ESHRE pages. Data will be collected in an anonymous way so that no individual data of centres are visible.
