WeCONNECT wants to hear from you!

Who is WeCONNECT?
WeCONNECT is a non-profit organization (operationally known as the Windermere District Social Service Society) in the Columbia Valley that is committed to inclusion by providing a variety of supports and services to adults with diverse abilities.

WeCONNECT is looking for your feedback to accurately identify needs, gaps and priorities in our community for those with diverse abilities now (1-5 years) and in the future (5+ years).

Who should complete the survey?
This survey should be completed by individuals with diverse abilities and who live in the Columbia Valley (Canal Flats to Spillimacheen) or a person who represents them such as a family member, caregiver or guardian. Please only complete one survey per individual. Please do not fill out a survey, unless you have been asked to do so by the individual or individual’s family/guardian.

What will happen with this information?
WeCONNECT acts in accordance with the Freedom of information and Privacy Act (FOIPPA) and the Personal Information and Privacy Act (PIPA). All responses will remain confidential with our consultant. Individual responses will not be shared. Personal information collected is to ensure an accurate assessment of community needs.

1. RESIDENTIAL SUPPORT
a) Cluster Living
Ongoing support is provided to a group of individuals who have homes in close proximity to one another (typically within the same apartment building). May also receive targeted hourly support through one-on-one or group arrangements.

A shared living arrangement in which the contractor controls the home through ownership, lease, or rental. Could also be described as a shared living arrangement in which the individual and contractor have established a joint tenancy agreement.

Requires CLBC to establish a contract with a qualified professional to address behavioural support needs. Services include consultation, capacity development and assessment.

3. RESPITE FOR FAMILIES
a) Contracted Respite
Respite coordinated by CLBC or by a community-based agency. Support is provided to individuals within their home, in the service provider’s home or in community.

a) Employment
Community Inclusion service that uses a variety of methods to ensure individuals achieve real work for real pay within an integrated setting. Services are goal-based and time-limited.

A Host Agency manages the Direct Funding clients receive from CLBC and helps them set up and manage supports that will assist in meeting their goals. With a Host Agency Agreement clients have a say in who works with them and what supports they receive.
For example: WeCONNECT serves as a Host Agency for individuals who receive direct funding from CLBC. WeCONNECT takes on the responsibility of record keeping, and will help choose and hire people to support the client.
