Dear BCM Colleague,

As part of our joint venture, Baylor College of Medicine and CHI are planning a robust telehealth/telemedicine outreach initiative to advance professional education and healthcare services reaching providers and patients at a distance. The purpose of this initiative is to foster and support relationships with clinicians in the Clinically Integrated Network and the CHI Texas Division for the purpose of advancing quality, value, and outcomes. 

We are writing on behalf of the planning committee at BCM and CHI to enlist your assistance in informing this effort and its priorities. The link below is to a brief survey to capture your current activities and to identify opportunities that you are interested in pursuing.  We have provided definitions of telehealth/telemedicine within the survey, which may be helpful to you in considering which, if any, of your current or future activities in education, consultation, and outreach are appropriate to include in your responses.

You can take the survey anonymously; however, if you are agreeable to being contacted in the event that follow-up is needed, we provide an opportunity to include your contact information.  We ask that you respond on or before March 27th, 2017
