This form must be submitted 60 days before the event by all event organizers.

Food Vendors: All vendors must submit the Temporary FOOD VENDOR Notification Form for Special Events and review the Special Events: Guidelines for Organizers and Vendors document.
Personal Services Vendors: All vendors offering personal services, including face-painting, must complete the Temporary PERSONAL SERVICES VENDOR Notification Form for Special Events and review the Personal Services at Special Events: Infection Control Guidelines for Vendors document. Those offering face-painting must also review Face Painting at Special Events: Basic Requirements for Vendors document.

After this notification form has been submitted, the person submitting this form will ONLY be contacted before the event if the public health inspector has any questions/concerns about the information listed on the notification form.
