School HVAC Assistance Form

This program assists Michigan K-12 public schools with recommendations to reduce infectious aerosol transmission via the heating, ventilating, and air-conditioning (HVAC) systems. Preference will be given to Environmental Justice Communities.

Michigan K-12 public schools that complete the K-12 Public Schools HVAC Survey (Link) are eligible to request free assistance with recommendations to reduce infectious aerosol transmission from an approved licensed HVAC contractor (or the school’s existing licensed HVAC contractor). The contractor will complete an HVAC inspection checklist to make recommendations, including cost estimates for implementation. Both the school building to be inspected and the HVAC contractor completing the inspection must be pre-approved by EGLE Energy Services (Link) prior to receiving assistance under this program. This is to ensure program integrity, and to help as many school districts as possible. [Note, the school’s existing licensed HVAC contractor can be approved by completing the Licensed HVAC Contractor Approval Request Form for this program (Link).]

I understand a completed K-12 Public Schools HVAC Survey (Link) must be received by Energy Services to be eligible for assistance under this program.

I understand both the school building to be inspected and the licensed HVAC contractor completing the inspection must be pre-approved by Energy Services.

I understand verification must be provided approving the licensed HVAC contractor to complete the onsite inspection and the onsite inspection was conducted by the approved licensed HVAC contractor.

I understand a copy of the completed HVAC inspection checklist and associated recommendation report will be provided to Energy Services upon request.

I agree to complete a brief follow-up survey evaluating the value of assistance received under this program.

I understand Energy Services my limit assistance under this program to one building per school district.
