CONSENT FORM

You are invited participate in a research study exploring school-based, occupational therapists perceptions and practices when supporting children who have sensory processing disorders.

Please read this following consent form carefully before agreeing to participate in this study.
 
The beginning of the survey will ask for basic demographic information and the remainder of the survey will consist of closed and open-ended questions regarding your intervention practices. 
 
This survey will take you approximately 15 minutes to complete.   
 
Voluntary Nature of the Study: This survey study is completely voluntary, and your participation is not required.  If at any point of the study you wish to withdraw or not answer a question, you may do so without any consequences. 

Risks and Benefits of Being in the Study: As with most research studies, you do face some possible risks.  You may experience some levels of stress due to the time commitment that is required to participate in this study while having to still meet your other commitments or obligations.  Remember that you are able to stop answering the questions and end the survey at any time.   
 
Compensation: No monetary rewards or other forms of compensation will be provided to the participants who agree to take part in this study.
 
Confidentiality: All of your responses and information will be kept confidential.  There will be no identifying information to be collected such as your name, email address, or IP address.  Investigators will keep all data stored in a password protected electronic format.  In efforts to protect your confidentiality, the surveys will not include any personal information that will identify you.  All of the results of this study will solely be used for scholarly purposes.  All data will be stored in a password-protected document within a password-protected laptop computer.  Only the primary investigator and co-investigator whose names appear on this study will have access to the files.

I have read the above information. I have asked any and all questions I may have had in regard to this study and was I given adequate answers to the questions I may have asked. I consent to be a participant in this study. I understand that I can withdraw from participation in this study without any consequences at any point.
