You must provide all information requested on this form for your child to be eligible to have their name entered in the lottery drawing. Incomplete registration forms will not be accepted. If your child is selected in the lottery drawing a parent/guardian will be notified by phone/email/text. Children not selected in the lottery will be placed on the waiting list for one year and will be contacted if spaces become available during the school year. If no spaces become available during the school year, please re-apply next year.
