This information sheet has been written to help you decide if you would like to complete the following pre-screening survey, therefore please read it thoroughly.

What would I be required to do?

- You will be asked to complete the current pre-screening questionnaire to ensure you meet our inclusion criteria. This will take approximately 5-10 minutes to complete.

This study has received ethical approval from the NRES Committee Yorkshire & the Humber (ref: 09/H1307/7; date: 02/04/15). The Principal Investigator of this research is Dr. Graham Finlayson (g.s.finlayson@leeds.ac.uk).

I understand that access to data will be limited to researchers involved in the project and that material gathered during this survey will not be made available to me.

I understand that my data from this survey will be stored for a period of 3 years before being destroyed.
