INTRODUCTION
I invite you to participate in a research study. I am a doctoral learner at Capela University in the School of Business, Technology, and Health Care Administration. I am conducting this research to complete my degree. I will write a paper about the research findings. This form will help you decide if you want to participate in my research. Participation in this research is voluntary.
If you have questions about the research, please ask me. Please let me know if you would like more time to think about your participation. You may also want to discuss your participation with people close to you.

WHAT IS THIS STUDY ABOUT?
I want to learn about what aspects of innovation might make an airline more attractive to pilots when they decide which airline to apply to fly for. This research may help future researchers or airline leaders better understand how to attract pilots to the profession.

WHY WOULD YOU WANT TO PARTICIPATE?
You might want to participate because your thoughts about innovation in your current employer may help identify ways for airline leaders to attract additional applicants and ease pilot shortages.

WILL PARTICIPATION HELP YOU OR OTHERS?
Participation in this study will not directly help you. Information from this study might help researchers or others in the future.

WHY WOULD YOU NOT WANT TO PARTICIPATE?
You might not want to participate because of:
-       risks to you confidentiality – There are risks any time you share information on the internet, and I cannot completely eliminate this risk since I cannot validate the security and anonymity of all devices and networks you might use to take this survey. It is, however, unlikely any person or organization is monitoring for information in this survey. To minimize the chance of somebody seeing the data who should not:
o   I am using a secure platform to collect the information,
o   I am not collecting personal information,
o   I will store the collected results on an encrypted hard drive for seven years and then destroy the data,
o   I will use a password-protected computer with an encrypted hard drive when viewing and evaluating the data,
o   I will not report any identifiable information in the resulting analysis.

-       Boredom or a Lack of interest in the research – While this survey should take approximately 15 to 25 minutes to complete, the style of questions might be considered monotonous by some participants. You may stop participating at any time.

Except for your time and inconvenience, there are no additional risks to you from participating in this study.
