CDC+ Satisfaction Survey

The Agency for Persons with Disabilities (APD) is conducting a Satisfaction Survey of the Consumer Directed Care Plus (CDC+) program.  Each participant in the program is being mailed the attached survey in order to obtain input about the program.  Your satisfaction with CDC+ is of great importance to us and we need your input.

Please complete and return the survey by September 18, 2017.

Please do not mail or fax the survey to us if you have completed the survey online.

The Satisfaction Survey should be completed by the participant, if possible, or by the participant’s representative if the participant is unable to complete the form.

All surveys will be confidential and anonymous unless you choose to provide your name.  Results will be reported only for the group, not for any individual, even if you do choose to include your name.  You will not be contacted regarding your responses to the survey unless you request to be contacted.
