You are being asked to participate in a survey that asks questions about how you think about learning and about your school and classroom environment.
Your answers will be combined with those of other students in your school and/or district to describe what students think, do, feel, and experience. This important information will help us understand how to better help students succeed in school.
You can always ask your teacher for help if you are having trouble understanding a question. You do not have to answer any question that you do not wish to answer, but we hope you will answer as many questions as you can. This is your chance to help improve your school-- don't pass it up!
