Overview

Thank you for participating in one of the Strengthen & Enhance Epilepsy Knowledge (SEEK) Training modules created by the National Coordinating Center for Epilepsy (Center) at the American Academy of Pediatrics (AAP). The goal of the SEEK Training is to build the capacity of those that may care for children and youth with epilepsy (CYE) to manage patients with considerations around the medical home, cultural competency, and shared decision-making.

Please complete this brief survey to provide feedback on the usefulness of the training module and to identify areas of opportunity.

The records of this project will be kept confidential. The survey is anonymous, and no personal identifiers will be collected. In any sort of report of findings, we will not include any information that will make it possible to identify specific participants. Records will be stored securely, and only AAP project staff will have access to the records.

This program has been reviewed and approved by the AAP Institutional Review Board (IRB). The risks involved with completing this survey are no greater than the risks a person may find in their daily life. You do not have to answer any question that you do not wish to answer, and you may stop completing the survey at any time.
