The Southern Black HIV/AIDS Network is a project of the Southern Black Policy and Advocacy Network, based in Dallas, Texas.

This survey is part of our effort to assess the capacity of advocacy for HIV/AIDS-related public health policy in the U.S. South, and then to develop advocacy strategies to be implemented in 2020 and 2021 on behalf Black communities. The project focuses on nine Southern states that have been most affected by HIV/AIDS.

This survey should only take 15-20 minutes to complete, and your responses are completely confidential.  Questions marked with an asterisk (*) are required.  All others are optional.

You have the option of providing your name and contact information if you want to join the Network.

You can only take the survey once, but you can edit your responses until the survey is closed on August 26, 2019.
