Sport Management – Midway Bike/Walk Survey
 
Dear Participant:
 
You are being invited to participate in a research study about the bikeability and walkability in the Midway Community. Your participation will involve completing an online/electronic or paper survey about your experiences navigating the Midway Community by bike or foot. You will also be asked to share your perceptions of safe biking and/or walking in your community. You will be asked some demographic questions. The information you provide will help us better understand the bikeability/walkability in your community. This study will focus on evaluating your experiences and feedback as a means of complementing and adding value to other measures utilized to evaluate bikeability/walkability in Midway. The online or paper survey will take approximately ten minutes to complete. You can skip any questions you do not wish to answer. The risks associated with this project are minimal and there are no direct benefits to you. However, your feedback and experiences may support greater health, wellness, fitness and/or travel options within the community. This research is supported by the Mayor’s appointed Midway Bike/Walk Taskforce.
 
Your name and other identifying information will not be collected. Your responses will be anonymous. “Your completed survey data in raw downloadable form will be stored electronically at Midway University on a password protected device for a period of 3 years following completion of the study, at which time all data will be destroyed by deletion”. Individuals from the Sport Management Program and the Midway University Institutional Review Board (IRB) may inspect these records.  In all other respects, however, the data will be held in confidence to the extent permitted by law.  Should the data be published, your identity will not be disclosed.
 
Your participation in this study is completely voluntary. By completing the electronic or physical survey, you are voluntarily agreeing to participate. 
 
You acknowledge that all your questions have been answered in language you can understand.  If you have any further questions about the study, please contact Dr. Jeb Gorham – jgorham@midway.edu – 859-846-6290.  If you have any questions about your rights as a research participant, you may contact the chairperson of the IRB at thighley@midway.edu. The IRB is an independent committee of university faculty, staff, and members of the community. It is established to protect the rights of individuals involved in research studies. You will be given the opportunity to discuss any questions about your rights as a research participant, in confidence, with a member of the committee.  Midway University IRB has reviewed this study.
