Supporting the Journey Home is a virtual education program for interprofessional community care teams working in First Nations communities
The program will provide practical tips and strategies on how to incorporate an early palliative care approach from a First Nations perspective.

Who is this for?
Eligibility for this program includes providers working in First Nations, Inuit, and Métis (FNIM) communities across Canada who support community members living with a serious illness. E.g. Physicians, nurse practitioners, nurses, social/mental wellness workers, patient coordinators, personal support workers, community health representatives, cultural workers, Elders. Anyone involved in the care of FNIM community members experiencing serious illness.
When
We will be offering two sessions (on Tuesdays and Thursdays from 12pm-1:30pm EST) every week starting January 13, 2026. Participants only need to attend one session per week, but can choose whichever one works best for their schedule.
Deadline
Spots are limited. Apply by January 5, 2026, applicants will be notified about their selection by January 9, 2026.
