To complete a valid nomination, please have the below prepared to include in this form:

1. Statement of Community Contributions:
Outline the nominee’s service to the University community and/or broader community in a voluntary capacity and set out the reasons for their involvement. Nominees are asked to distinguish between voluntary community involvement and mandated or incentivised University requirements, such as internships, subject’s assessment activities and/or leadership roles. The statement should include specific tasks or actions that have been undertaken, length of service and outcomes achieved. (350 word maximum)

2. Statement of Academic Achievement at the University of Wollongong: Highlight the nominee’s most notable academic achievements including any prizes or scholarships, contributions to their field of study, or academic excellence relative to their academic journey. The University will provide the Selection Committee with an up-to-date enrolment record and WAM calculation. (350 word maximum)

3. Statement of Leadership Qualities and Ambassadorship Capability:
Outline the leadership qualities that the nominee possesses, including specific examples of when these qualities have been demonstrated. Details of the nominee’s contribution to the University community and/or the broader community should also be included. It is also necessary to explain how these qualities will enable the nominee to act as an ambassador for the University. (350 word maximum)

4. Statement in Support of Nomination (Optional) (200 word maximum)
