Welcome to our Vaccine Study Questionnaire

Apex Trials is conducting a study to investigate the immune response and safety of vaccine against respiratory syncytial virus (RSV) given to adults 18 to 49 years of age at increased risk for respiratory syncytial virus disease, compared to older adults 60 years of age and above.
The study will involve a total of 3 visits to our clinic (located in Guelph, Ontario) over a 6- month period. You will be reimbursed for expenses in the amount of $75.00 and you will also receive a stipend of $50.00 per visit.

If you are interested in participating in this study, the following questions will help us determine if you qualify. Please provide complete and honest answers to the questions to ensure this study is a good fit for you. All information collected will be kept confidential and will not be shared without your permission except as required by law. The questionnaire will take approximately 3 to 8 minutes to complete.
