Thank you for taking the time to participate in our Community Health Assessment survey.  The survey will take 7-10 minutes to complete.

This survey is intended only for people 16 and older who live or work in Asotin County. 

There are several open response opportunities for you to share your thoughts but those are optional. 

Information from this survey will be used to help Asotin County Health District develop and improve upon programs as we increase our presence in the community. 

All information is anonymous and you will not be asked for your name, address, email address, etc. at any time.
