Woodcock-Johnson Research Study Permission Form v.2207

Riverside Insights is recruiting examinees from 3 through 80+ years of age to participate in our national research studies involving the Woodcock-Johnson cognitive, educational, and oral language assessments. Examinees selected to take part in a research study will be administered a series of test items on an individual basis. Total testing time and compensation amount are determined by the study for which the examinee is selected. Riverside will keep all testing results confidential. Because this is a research project, Riverside will not provide feedback on test performance. Information collected will be used only for research purposes.

To be considered for this opportunity, please complete the survey questions below. We will use the information you provide about your (or your child's) age, location, race/ethnicity, education, and/or occupation to determine whether you (or your child) are a candidate for this study. The information you share with us, including your contact email, will be distributed to Field Examiners on our Research team for the purpose of assigning examinees to Field Examiners, but will not be sold, and will not be distributed outside of Riverside Insights' Research team. If you do not wish to share your (or your child's) personal demographic information, you may exit the survey now by closing your browser tab. This survey will take approximately 5 minutes to complete.

If you (or your child) are selected to participate in the research study, your Examiner or Recruiter will reach out to you with additional details (including the compensation amount for your participation) and to schedule one or more testing sessions. Please note that completion of this survey does not guarantee participation.
