RHIO/QE Connections and Barriers Survey

Thank you for participating in this important survey measuring connecting to Regional Health Information Organizations (RHIO)/Qualified Entities (QE) and the potential barriers in New York State (NYS). This survey is being conducted by the New York State Office of Mental Health (OMH). RHIOs/QEs are certified regional health information organizations that are regulated entities associated with the Statewide Health Information Network for New York (SHIN-NY) who facilitate clinical health information exchange. Each RHIO/QE has regional presence but can support participants anywhere in NYS.

Today we will be gaining your thoughts and opinions in order to better serve you in Health Information Technology development. Please answer each question to the best of your ability, making estimates if necessary. Your information will be shared with the local RHIOs/QEs to better serve you in the health information exchange in the future.

Please feel free to share this survey with any staff more suited to answer these survey questions.
