New Client R.E.A.L. Assessment

Thank you for taking time to complete this initial client assessment which will be used to assist you with meeting your personal career goals. You and I will discuss your results at your consultation if you choose to schedule one. The information I collect will be held in the highest of confidentiality so please feel free to answer all the questions honestly and completely. There are no right or wrong answers because there are no right or wrong versions of you. Do not think too hard about how to answer any of the questions because they are designed to collect data that will create the best depiction of who you are. Keep in mind that usually the first answer that comes to your mind is the correct answer.

This survey should take approximately 15 minutes to complete.  Of course some may take longer than that time to complete it, and some may complete it sooner.  The important thing is to answer the questions honestly and to the best of your ability.
