An Overview of Work-Related Asthma for Primary Care Providers Post Test, Evaluation & Credit Form

Thank you for participating in the An Overview of Work-Related Asthma for Primary Care Providers provided by Tulane University Center for Continuing Education, Tulane University School of Public Health's Department of Global Environmental Health Sciences, and the Association of Occupational & Environmental Clinics.

This electronic survey will allow you to provide your evaluation of this educational activity in addition to requesting your AMA PRA Category 1 Credits™, CEUs, or a Certificate of Participation.

It is important for the Tulane University Center for Continuing Education, Tulane University School of Public Health's Department of Global Environmental Health Sciences, and the Association of Occupational & Environmental Clinics to evaluate the educational impact on the participant. We encourage you to complete the course evaluation regardless of your desire to claim AMA PRA Category 1 Credits™ or CEUs.

The next steps are to reflect on your experience, as well as evaluate the course and the impact you predict it will have on your practice and/or research.

There will also be an optional section for physicians to claim their AMA PRA Category 1 Credits™ and non-physicians to request CEUs or a Certificate of Participation.

Requests for a certificate of credit or participation requires a successful (70%) completion rate of a post-assessment.

A $25 certificate fee will be charged. At the end of this form, you will be given payment instructions.

The course evaluation is submitted and downloaded into aggregate data to maintain your anonymity. Your request for AMA PRA Category 1 Credits™, CEUs, or a Certificate of Participation must include full contact information, however, and is processed separately. This site is SSL protected and all downloads are password protected.
