Pastor, it is vitally important that all individuals who have been granted a Local Minister's License are registered on the district so that we have this person's name and contact information in our records. To help us accomplish this, we ask that you please fill out the following registration/information form. Please note that you need to fill out a separate form for each local licensed individual in your church. This form generates a document that will be placed in the candidate's file.
