Participant Information

Introduction
Lady Cilento Children’s Hospital Pharmacy is conducting this survey to investigate parents’ and carers’ experiences with finding information on the medicines they are giving to their children.

Background
There has been published research on adult patients’ needs for medicines information however there is limited data on that for children. This study aims to identify parents’ and carers’ expectations and needs on medicines information; such as type of information required, when it is required and how to access them. Medicines information tailored to the needs of parents and carers will ensure safe and appropriate use of medicines at home.

Methods
Please fill in this survey questionnaire. It will take approximately 5-10 minutes to complete.

Risk & Discomfort
We do not foresee any potential discomforts or inconveniences.

Benefits
Your response will guide us and other relevant organizations on how best to prepare paediatric medicines information and to make it available to you.

Withdrawing from the Study
Your decision whether or not to participate will not prejudice your children’ future relations with the Lady Cilento Children’s Hospital (LCCH). If you decide to participate, you are free to withdraw your consent and to discontinue participation at any time. The decision to withdraw from the study will not affect their routine medical treatment or their relationship with the people treating them.

Confidentiality
Your responses will be treated confidentially. We do not collect any identifying information in the survey questionnaire. The data may be accessed by auditors, ethics committee or regulatory authorities and the results may be published.
