Application Deadline (EXTENDED): Wednesday, July 17, 2019, by midnight. 
 
Application Process. In collaboration with the Administration for Community Living, the National Council on Aging will select up to 10 organizations to participate in this ten-month learning collaborative. Selection will be based on review of applications. Organizations should coordinate with others in their designated coverage area or state and identify one organization as the lead on the learning collaborative initiative.
 
Eligible applicants. This learning collaborative is designed for organizations that are in the early to intermediate stage of developing their business acumen and organizational strategies.  The ultimate goal for these community-based organizations is the development of peer networks to create local, regional, and/or statewide community-integrated networks for the provision of long-term services and supports services, including evidence-based health promotion and disease prevention programs. These community-integrated networks will provide complementary services to be packaged and marketed to health care organizations to advance the cause of fully integrated care in your communities.
 
We are seeking a diverse group of participants for this collaborative to foster peer support and practical experience to augment the training/technical assistance that will be provided. Applicants with the following characteristics are encouraged to apply:
  • Organizations that have just begun to examine opportunities to contract with health care organizations.
  • Networks that have already begun examining local partnership opportunities and community-integrated network partners.This should include the exploration of centralized and coordinated processes to expand bandwidth to contract with health care organizations.
Applicants and their partners must be committed to dedicating 4-5 hours per month to actively participate in this learning collaborative (see “Participant Expectations” in the Network Development Learning Collaborative Charter).
 
Organizations that are NOT eligible to participate. Those who are participating in or who have been selected to participate in a concurrent business acumen learning collaborative with an overlapping time frame; or previous completers of the Network Development Learning Collaborative.
 
Monthly NDLC webinars will be held on the 3rd Tuesday of each month from 3:00 pm to 4:30pm ET from September 2019 through June 2020.
