VENDOR APPLICATION FOR EXHIBIT SPACE

You are invited to join FHSA as an Vendor/ Exhibitor for the 2016 Annual Conference to be held at the Hilton Daytona Beach Resort, April 20-22.  The Exhibitor Showcase will be held Thursday, April 20 from 9:00 a.m.- 5:00 p.m. We invite our Exhibitors to join us for the Wednesday evening, April 20, for the Wednesday Night Mixer. This is a great opportunity to meet and network with the conference attendees. Additionally, we ask that our Vendors/ Exhibitors provide door prizes that will be given away during the Thursday Exhibitor Showcase and also are welcome to provide items for the Conference Attendees Bags.

The Vendor/Exhibitor Application should be submitted no later by January 29, 2016.

FHSA is interested in identifying corporate partners who want to serve as a Sponsor for this year's conference.  The first level of sponsorship is $1,000 and includes 2 Vendor/Exhibit Tables, 1/4 page ad in the Conference Program and an opportunity to place items in the Attendees Conference Bag. If you are interested in providing a Sponsorship at either the $1,000 level or higher (including additional benefits), please contact Chris Duggan, FHSA Consultant by email at chris@FLHeadStart.org or by phone at 850-694-6477. 

If you are providing a Sponsorship Contribution, we ask that you complete this form and within Question 11, mark the Sponsorship Contribution Exhibit Table option. The tables will be provided to you as a Sponsorship Contribution benefit and we will include your table(s) in the total count.

Please respond to all questions in the application to help us met your needs at the event. If you also want to provide a Workshop Presentation, you will need to submit a Presentation Application in addition to the Vendor Application.
 
If you are selected as a Vendor/Exhibitor for the 2016 Conference, you may pay the Vendor fee through PayPal or you may request an invoice from FHSA. Once selected, you will receive an email with the needed information including vendor/exhibitor fee that has been determined from this application.  If you are not selected, we will notify you and will be glad to discuss your application to see if our questions and/or concerns can be addressed so you can join us.

Vendor/Exhibitor Fees:

1. One Exhibit Table.....$ 500
2. Two Tables .............. $ 750
3. Three Tables......... .. $ 875
4. Sponsorship Exhibit Table (s).......No fee as these tables are provided as a Sponsorship Contribution Benefit
5. Resource Table Display ..... $125- Exhibitors who can not be present at the conference but would like to have a Resource table set up with resources for attendees.
6. Exhibitor-in-a-Bag ...... $100- Exhibitors who can not be present at the conference but would like to have items added to the Conference bags.
7, Show and Tell Session- There will be limited number of these sessions available.
         20 Minute Session- $100
         40 Minute Session- $200

It is important that everyone attending the Conference be entered into the Conference Registration system so that they will receive communications about the event and receive a name badge and materials upon arriving at the Conference. if you are selected as a Vendor/Exhibitor, you will be asked to register as an Exhibitor on the Conference Registration website.

NON-PROFIT ORGANIZATIONS: Please see Question 2 if you would like to request a complimentary display table to distribute educational information only to Attendees at no cost. (Skip Q9 and 10).
