Tá an fhoirm chlárúcháin seo á reáchtáil ag Oireachtas na Gaeilge thar ceann na Roinne Forbartha Tuaithe agus Pobail agus Gaeltachta (DRCDG) trí mheán SurveyMonkey, córas tríú páirtí. Beidh aon eolas pearsanta a chuireann tú ar fáil mar chuid den fhoirm chlárúcháin seo á stóráil agus á phróiseáil ag SurveyMonkey ar feadh tréimhse ghairid. Tá cóip d’Fhógra Príobháideachais agus Fianáin SurveyMonkey le fáil ag bun an leathanaigh ar chlé.
