iNICQ 2017: Choosing Antibiotics Wisely - Team-Based Data Self Assessment

1. Instructions

One of the key goals of the iNICQ 2017 is to implement the CDC Core Elements of antibiotic stewardship and develop an enduring antibiotic stewardship program that will sustain and serve your unit long after this “project” is complete. A secondary goal for Intensive centers is to explore the process of reporting this data to the NHSN thereby creating a long-term data source that will serve both your unit and the neonatal community over time.This brief survey is designed to provide your team with a "snapshot of your current state.
