COVID Vaccine Provider Application/Registration and Checklist for Physicians

1. This application/registration is for the COVID-19 vaccine ONLY.

2. For vaccine information, refer to online COVID-19 immunization resources at https://formulary.drugplan.ehealthsask.ca/COVIDImmunizationProgram and the online COVID-19 Immunization Manual available at https://www.ehealthsask.ca/services/Manuals/Pages/COVID-19.aspx

3. Physician clinics may receive Moderna vaccine or Pfizer vaccine.

4. Record the COVID-19 vaccine administered on the line lists available at https://www.ehealthsask.ca/services/Manuals/Pages/COVID-19.aspx and submit daily.

5. If approved, Medical Services Branch will connect your clinic with the SHA local public health office for the purpose of ordering and distributing the COVID-19 vaccine.
