2025 Survey: Health and Wellness in Santa Clara County Asian and Pacific Islander Communities

Introduction

The Asian Pacific Islander Community Health Worker Network in Santa Clara County is conducting a health assessment of Asian and Pacific Islander communities in the county. This assessment will help the network and funders understand the health needs of these groups. The information we gather from this survey will be used to strategically allocate resources, plan services, inform program development, and address health and social inequalities and disparities.

As part of this process, we are surveying people who meet the following criteria:
  • Residents who live in Santa Clara County
  • Ages 18 and older
  • Identify from one or more API subgroups (see table below for more details)
Asian Pacific Islander
  • Asian Indian
  • Chinese
  • Filipino
  • Japanese
  • Korean
  • Vietnamese
  • Native Hawaiian
  • Guamanian or Chamorro
  • Samoan
  • Pacific Islander
  • Polynesian, such as Tahitian, Tongan, and Tokelauan
  • Micronesian, such as Marshallese, Palauan, and Chuukese
  • Melanesian, such as Fijian, Guinean, and Solomon Islander

Your responses will be anonymous and confidential. The survey does not ask for your name or any identifying information and no responses will be attributed to any particular individual. Your participation in this survey is completely voluntary. You can skip any questions or decline to participate at any point.

The survey should take approximately 15 minutes to complete.
