  1. I have no outstanding annual subscription and practising certificate fee due to the Institute;
  2. I have complied with the Institute’s CPE requirements;
  3. I have no disciplinary cases or pending investigation case with the Institute; and
  4. I have read the PDPA and Privacy Notice of the Institute at the Institute’s website and hereby give consent to the Institute to provide the above details to the PLC.
