Asking you about telehealth

Telehealth is the term for receiving your doctor and other healthcare services (e.g. speech services, occupational therapy, applied behavioral therapies, etc.) through a computer, tablet, or smartphone using videoconferencing.  You may have used videoconferencing when you used Skype or FaceTime with your friends.  Sometimes telehealth is done over the telephone especially if a videoconference cannot be done.

We are participating in a project to develop resources to help families use telehealth.  This survey will help us find out what help families need to use telehealth well.

After you complete the survey, you will receive one (1) entry into a drawing for a $50 Amazon gift card.  To enter the drawing, you will be asked to provide your email or phone number on a separate page that is separate from your survey answers.  Your email address or phone number will be kept confidential and will only be used for the random drawing for the gift card and destroyed after the drawing.
